What we collect, why we have it, and what we will never do with it. In plain words.

Who we are
Injury Centers of Texas (14200 Gulf Freeway) and PHW Southwest (8323 Southwest Freeway), two physical medicine clinics in Houston under the same management. This page covers this website, our online paperwork, and the text messages and emails we send.
What we collect
Only what we need to treat you and get you paid for.
- What you type on our forms: your name, date of birth, contact details, how you were hurt, your employer or the other driver, your insurance or claim details, and your attorney if you have one.
- What you tell us at the front desk or on the phone, written into the same record.
- Basic technical information every website receives, like which pages were opened and from what kind of device.
Why we have it
To treat you, to schedule you, to complete the paperwork a workers' compensation or auto claim requires, and to bill the party responsible for paying — which for the patients we see is almost never the patient.
What we will not do
We do not sell your information. We do not rent it. We do not hand it to advertisers or data brokers.
We do not sell or share mobile opt-in information or phone numbers with third parties for marketing purposes. Your number is used to reach you about your own care and nothing else.
Who does see it
Only the people who have to, in order for you to be treated and for the claim to be paid.
- Clinic staff involved in your care.
- The insurance carrier, adjuster, or employer handling your claim — because that is who pays, and they require the records to do it.
- Your attorney, if you have one and you told us who they are.
- Companies that run our software and send our messages on our behalf, under agreements that hold them to the same rules we follow.
- Anyone a law or a court order requires us to give it to.
Your medical records
Your medical records are protected health information and are handled under HIPAA and under the clinic's Notice of Privacy Practices, which you are given when you become a patient. This page does not replace that notice — where the two differ, the notice governs your medical records.
Text messages and email
We only text people who ticked the consent box on our intake form. Our system refuses to send to any number it has no record of permission for, so if you never ticked it, you will not hear from us by text.
Reply STOP to any message to stop them, or tell the front desk. Every email we send carries an unsubscribe link. Stopping either does not affect your care.

How long we keep it
Medical and billing records are kept for as long as Texas law and the claim require, which is usually several years after your last visit. We do not keep things longer than we have a reason to.
Children
This website is not directed at children, and we do not knowingly collect information from anyone under 13 through it. A minor treated at the clinic is treated with a parent or guardian involved, and their record is handled the same way every patient record is.
Asking us about your information
Call (281) 857-4989 or email Referrals@phwsouthwest.com and ask. You can ask what we hold, ask us to correct it, or ask for a copy of your records. We will tell you what we can do and what a law requires us to keep.
Changes
If this page changes, the date at the top changes with it.
